Adult-Gerontology Nurse Practitioner · Founder & Owner of Vital Haven Wellness

20+ Years in Healthcare

Hi, We’m Heather Stevens, MSN, AGNP-C, a board-certified Adult-Gerontology Nurse Practitioner, and founder of Vital Haven Wellness.

We am a Member of The Menopause Society and have completed certification and training in men’s and women’s hormone replacement and testosterone therapy.

We created Vital Haven Wellness for adults who want care that starts with listening and looks at the whole picture — whether you’re navigating hormone changes, concerns with weight or metabolism, changes in energy, or simply want to take a more proactive approach to your health.

Care begins with understanding your health history, symptoms, and laboratory findings, with clear information to help you decide what feels right for you.

Our mission is to give you the time and support you need to live well, stay active, and continue doing the things that matter to you.

Fatigue, poor sleep, changes in weight or body composition, low libido, brain fog, mood changes, decreased motivation, or simply not feeling like yourself can significantly affect your quality of life. These concerns deserve a thoughtful evaluation rather than being automatically dismissed as part of getting older.

If you know something has changed in how you feel, it deserves to be explored.

Our Approach to Care

Holistic, evidence-informed, and patient-centered

There is rarely one symptom, laboratory value, or guideline that tells the entire story. We consider the complete clinical picture — your symptoms, medical history, medications, laboratory findings, risk factors, metabolic health, sleep, lifestyle, and personal goals.

Our role is to bring those pieces together, consider other possible contributors to how you are feeling, and help you understand your options.

Care Built Around You

During your visit, we will talk openly about what you are experiencing, review your health history and relevant laboratory findings, and discuss appropriate options in clear, understandable language.

When more than one reasonable treatment approach exists, we review the potential benefits, risks, limitations, and alternatives with you.

Through shared decision-making, you and we develop a plan that is medically appropriate, realistic for your life, and aligned with your health goals.

Treatment is not about chasing a predetermined number. It is about evaluating your overall clinical picture, monitoring your response, and making thoughtful adjustments when appropriate.

Areas of Care

What Vital Haven Wellness offers

Hormone Optimization

Personalized hormone evaluation and treatment for women and men experiencing symptoms that may be related to hormonal changes.

  • Perimenopause and menopause care
  • Women's hormone therapy
  • Men's testosterone replacement therapy
  • Evaluation of hormone-related symptoms
  • Ongoing clinical and laboratory monitoring

Treatment decisions are based on the individual patient rather than a single laboratory value or standardized protocol.

Medical Weight Management & Metabolic Wellness

Weight and metabolic health are influenced by many factors, including hormones, medications, nutrition, activity, sleep, medical conditions, and individual metabolism. Your evaluation considers your health history, laboratory findings, previous weight-management efforts, metabolic health, and personal goals.

Medication selection is individualized based on your health history, treatment goals, potential risks and benefits, previous response to treatment, and what is medically appropriate for you.

Supportive Wellness Therapies

Vitamin, nutrient, and other supportive wellness injections may be incorporated into your care when clinically appropriate and aligned with your health needs.

These therapies are considered part of your broader treatment plan and are selected based on your individual needs and goals.

Lab-Informed Wellness Care

Laboratory testing can provide valuable insight into hormone health, metabolic function, nutritional status, and other factors that may contribute to how you feel. Testing is selected based on your individual clinical needs and interpreted in the context of your symptoms, medical history, and overall health.

Laboratory testing may include an in-house blood draw or an order for an outside laboratory when appropriate.

Feeling different as you age does not mean you should simply accept feeling depleted.

Hormonal changes, metabolic health, sleep, stress, nutrition, lifestyle, medications, and other health factors can all influence how you feel.

Our goal is to identify what may be contributing to your symptoms, determine what can appropriately be addressed, and help you move toward feeling healthier, stronger, and more like yourself.

A Plan That Fits the Life You Actually Live

There is no copy-and-paste approach at Vital Haven Wellness.

Whether your care includes hormone optimization, medical weight management, metabolic wellness, supportive therapies, lifestyle changes, or a combination of approaches, your treatment plan is developed around what is appropriate for your health and realistic for your life.

Care continues beyond the initial visit. We reassess your symptoms, clinical response, relevant laboratory findings, and overall health so your treatment plan can evolve as your needs change.

Most importantly, we want you to understand your options, know why a recommendation is being made, and feel confident about the next step in your care.
